Stabilising a “Package at Risk” Through Safer Transfers

A long-standing home care package became increasingly fragile following several hospital admissions, as the client’s functional baseline declined; particularly their ability to weight-bear and participate in transfers safely.

The client had been transferring with a standing hoist for several years, but over time engagement reduced, strength deteriorated, and they began spending increased time in bed. Two carers reported that transfers were becoming progressively difficult, with growing reliance on physical lifting to “get the transfer done.” Several near-misses were documented, the family became understandably anxious, and the agency was concerned about client safety, carer injury risk, safeguarding escalation, and the risk of the package breaking down.

The Occupational Therapist intervened promptly to reassess the transfer approach and equipment. Following a moving and handling assessment in the client’s home, it became clear the standing hoist was no longer the safest option. A mobile hoist with an appropriate sling solution was introduced, alongside a clear, practical manual handling plan and risk assessment aligned to the agency’s working practices. A hands-on demonstration was delivered with the care team during the follow-up visit in the client’s home environment, ensuring correct positioning, sling application, and a consistent method carers could replicate shift-to-shift.

Transfers became safer and more predictable, reducing strain on carers and restoring confidence for the client, family, and staff. Importantly, the client was able to sit out of bed again and spend time in their riser-recliner chair as part of their daily routine. With a defensible plan and the right equipment in place, the care package stabilised and continued safely.
